About Jane Cranston

Founder, Tiny Transitions Therapy Consulting PLLC

Tiny Transitions Therapy Consulting was inspired by both her personal journey as a mother and her professional work as a pediatric occupational therapist. As the parent of a child with special needs, she understands the challenges and hopes families face. Raising a deaf child, ASL was the primary language at her home—an experience that deepened her belief in the power of communication and connection.

With over 25 years in pediatric therapy, she has worked in NICUs, outpatient clinics, schools and homes, supporting infants and children with complex needs. She has helped families navigate early development, trained new therapists, and created programs that make care more accessible. She has developed and led innovative services to support babies after NICU discharge and guide families during the fragile transition from hospital to home.
Through Tiny Transitions Therapy Consulting, she offers services including developmental therapy, parent coaching, clinician mentorship, and program consulting. Her goal is to support families and professionals through each small but important step, because these early moments truly matter.
