Evidence-Based Occupational Therapy
At Tiny Transitions Therapy Consulting, every recommendation, intervention, and parent coaching session is grounded in the latest research and clinical best practices.
We combine scientific evidence, clinical expertise, and family values to create personalized support for each child and caregiver we serve.
What Evidence-Based Care Means to Us: Evidence-based practice isn't just a checklist—it's a way of thinking. We use proven tools and strategies that support brain development, sensory regulation, motor skills, feeding, and caregiver bonding. Our therapists stay current on emerging research and continuously adapt care plans to reflect what works best—safely and compassionately.
Our Approach Includes:
- Neurodevelopmental Care: Techniques that support healthy brain and body development from infancy through toddlerhood.
- Trauma-Informed Principles: We honor each family's story and take care to build trust and safety during all interactions.
- Responsive Feeding Support: Rooted in the latest pediatric feeding research, we guide families through sensory and developmental feeding challenges with gentle, paced strategies.
- Caregiver Collaboration: We view parents as active team members. Your observations, insights, and goals are essential parts of every plan.
- Ongoing Education: We regularly update our knowledge by attending and presenting at leading conferences such as NANT, AOTA, and more.
Why It Matters: Using evidence-based practices helps ensure that your child receives care that is safe and appropriate for their medical and developmental needs.
Research-Driven Therapy Approach
Our care is shaped by the latest findings in neonatal and pediatric therapy.
We stay current on evidence across early childhood development, trauma-informed care, and family-centered occupational therapy. Our therapy adapts to reflect both scientific progress and your child's evolving needs.
Through continuous professional learning and clinical experience, we ensure that every approach is research-aligned, gentle, and developmentally appropriate.
Family Support Systems
We provide families with tools and coaching to support development at home.
From feeding routines to sensory and motor strategies, we equip parents with practical methods for everyday life. Our team guides you in integrating therapy into daily routines—mealtimes, play, sleep, and transitions—so your child can thrive between visits.
These supports build consistency, encourage caregiver confidence, and reinforce developmental growth beyond the therapy setting.
Caregiver Collaboration
We see caregivers as essential partners in therapy.
We actively listen to your insights and observations, then integrate them into the therapy plan. Our collaboration ensures the care we provide reflects your child's real-world context.
Your voice matters. Whether it's setting therapy goals, adjusting strategies, or problem-solving challenges, we work with you every step of the way—because you know your child best.
About Jane Cranston
OTR-MOT, BCP, CNT, NTMTC
Founder, Tiny Transitions Therapy Consulting PLLC
Tiny Transitions Therapy Consulting was inspired by both her personal journey as a mother and her professional work as a pediatric occupational therapist. As the parent of a child with special needs, she understands the challenges and hopes families face. Raising a deaf child, ASL was the primary language at her home—an experience that deepened her belief in the power of communication and connection.
With over 25 years in pediatric therapy, she has worked in NICUs, outpatient clinics, schools and homes, supporting infants and children with complex needs. She has helped families navigate early development, trained new therapists, and created programs that make care more accessible. She has developed and led innovative services to support babies after NICU discharge and guide families during the fragile transition from hospital to home.
Through Tiny Transitions Therapy Consulting, she offers services including developmental therapy, parent coaching, clinician mentorship, and program consulting. Her goal is to support families and professionals through each small but important step, because these early moments truly matter.
Specialized Therapy Designed for Your Baby's Needs
Premature and medically complex infants require specialized therapy to support early development. At Tiny Transitions, we offer individualized developmental therapy that addresses motor skills, sensory processing, feeding challenges, and bonding — all tailored to your baby's unique medical history and developmental stage.
Our approach integrates neurodevelopmental principles, trauma-informed care, and feeding therapy techniques to gently promote progress while honoring your baby's cues. Whether your baby is newly home from the NICU or facing ongoing complexities, we help foster healthy development and stronger caregiver connections from the very beginning.
Clinical Development
At Tiny Transitions Therapy Consulting, we’re committed to developing the next generation of neonatal and pediatric therapists. We offer clinical education services including guest lectures, 1:1 and group mentoring, and continuing education designed for professionals working with—or hoping to work with—high-risk infants. With extensive experience in NICU and early intervention care, our training is grounded in real-world practice, trauma-informed principles, and a strong commitment to developmental care. Whether you're a student, new clinician, or an established therapist expanding your skillset, we provide the support and expertise to help you grow with confidence
